The ambulances had been strafed with machine gun fire rather than being targeted with missiles. The large hole in the centre of the red cross is where the red light was mounted. The machine gun fire shattered the plastic, leaving a central hole. I don't think that even US weapons targeting systems are good enough to hit a red cross dead-centre.
